Database insert checkbox value

i have a page "create.php/id=xx"
I have a Database like

Code: Select all

Id--quiz---answer1--answer2-answer3

I want to put all this quiz on page "create.php/id=xx" with checkboxes.
and then insert in database all quiz id that was checked.

Code: Select all

id-questionnaire----quiz_id

Please help me!
